Evaluations & Verification of Participation Forms

Dear Meeting Participant:

Thank you for your participation in the society's 2010 meeting(s). If you have not done so, please take the time to complete the meeting evaluation form. Your evaluation scores and comments are taken very seriously by the Scientific Program Committee when planning future meetings.

To receive your CME certificate, you must complete, sign and return your VOP form to the SCA's headquarters office (members see below to complet form online). Your certificate of participation will be mailed to you within 4-6 weeks.
